Patrick Eugene Formby, the son of Evelyn and Paul Formby, passed away on January 17, 2014 at the age of 86. He was born on November 11, 1927 in Floydada, Texas and had been a long time resident of Los Alamitos, California.
Patrick was preceded in death by his loving wife Ellen.
He is survived by his son Paul (MaryBeth) of Orange, California and his daughter Patrice Johnson (Paul) of Dakota Dunes, South Dakota, four granddaughters and two grandsons.
