Passed away peacefully with his family by his side. He is survived by his wife Shizuko Nakata, son Carey (Jessica) Nakata, daughter Kathy (Jon) Nomura, sister Sumako (Isao) Tanouye, grandchildren, great grandchildren, nieces and nephews. Per Chiaki's wishes, no services are planned.
